Zusammenfassung
Das Ziel der Diagnostik bei Multipler Sklerose ist die frühe und sichere Erkennung der Erkrankung, um eine zeitnahe und adäquate Therapie einleit en zu können. Des Weiteren dient sie der Verlaufsbeurteilung der Erkrankung mit dem Ziel einer an die Verlaufsform und Krankheitsaktivität angepassten und konsequenten Therapie. Zum Nachweis einer räumlichen und zeitlichen Dissemination von Entzündungsherden, d.h. dem Vorliegen von Entzündungsherden an mehr als einem Ort im Zentralnervensystem sowie dem zeitlich voneinander unabhängigen Auftreten von Entzündungsherden im Verlauf der Erkrankung, stützt sich die Diagnosestellung auf verschiedene Untersuchungsmethoden. Hierzu zählen die Anamnese und die klinisch-neurologische Untersuchung, die Magnetresonanztomografie, die Aufzeichnung evozierter Potenziale und die Liquoruntersuchung. Andere (entzündliche) Erkrankungen des Zentralnervensystems müssen ausgeschlossen werden. Keine dieser Untersuchungsmethoden ist jedoch allein dazu in der Lage, die Diagnose Multiple Sklerose zu stellen. Aus diesem Grund beruht die Diagnosestellung auf definierten Kriterien, die sich aus einer Kombination verschiedener Untersuchungsmethoden zusammensetzen. Die von der Deutschen Gesellschaft für Neurologie empfohlenen McDonald-Kriterien stützen sich sowohl auf klinische als auch auf paraklinische Tests und betonen insbesondere die Bedeutung der Magnetresonanztomografie im Rahmen der Diagnosestellung der Multiplen Sklerose. Auch im Zuge der Entwicklung der Diagnosekriterien hat die Diagnosehäufigkeit der Erkrankung in den letzten Jahren zugenommen, jedoch ist generell ein Anstieg der Inzidenz der Multiplen Sklerose in den letzten Jahrzehnten zu beobachten. Die Verlaufsbeurteilung der Krankheitsaktivität kann mit dem multifaktoriellen Multiple Sclerosis Decision Model erfolgen, das neben dem Auftreten von Schüben auch die Behinderungsprogression, neuropsychologische Aspekte und MRT-Befunde berücksichtigt. Auf Grundlage der vorhandenen Studien- und Datenlage sind präzise Aussagen zur Versorgungssituation in der Diagnostik der Multiplen Sklerose nur sehr eingeschränkt möglich. Die vorhandene Datenlage gibt Hinweise, dass die Versorgungskapazitäten mit MRT für Patienten mit Verdacht auf Multiple Sklerose ausreichend sind. Um eine exakte Bewertung der Versorgungs situation bezüglich der MRT-Diagnostik treffen zu können, sind weitere Untersuchungen notwendig, die fallbezogen sowohl stationäre als ambulante Daten berücksichtigen. In den letzten Jahren ist ein Trend zu verkürzten Diagnosezeiten vom Auftreten erster Symp tome bis zur Diagnosestellung der Multiplen Sklerose zu beobachten. Aktuell beträgt die durchschnittliche Dia gnosedauer in Deutschland 2,7 Jahre.
